Why it matters
CRE bloodstream infections carry 40–70% mortality. First large RCT to compare new beta-lactam/beta-lactamase inhibitor combinations head-to-head.
Key finding
Ceftazidime-avibactam achieved 28-day all-cause mortality of 24% vs. 34% for meropenem (OR 0.61, 95% CI 0.48–0.78). NNT of 10.
Limitation
Open-label design; 78% of patients had KPC-producing isolates, limiting generalizability to other resistance mechanisms (OXA, NDM).
